Hospital stay
If you are in hospital or a rehabilitation facility, you must inform us immediately.
Please submit proof of your inpatient stay to us immediately. If you have been given an appointment for inpatient admission, e.g., because you are undergoing rehabilitation, please submit a copy of your invitation to us.
