Hospital Stay
If you are in the hospital or at a rehabilitation facility, you must inform us immediately.
Please submit proof of your inpatient stay to us as soon as possible. If you have been given an appointment for inpatient admission—for example, because you are undergoing rehabilitation—please submit a copy of your admission notice to us.
